Important changes to our recall system for patients with long term conditions

If you are a patient with a long term condition which requires annual or regular reviews, please read this important notice regarding changes we are making to our recall system.
 
If you have consented to SMS text messages you will receive a link to a copy of this letter in the next couple of days.
 
Patients without a mobile number will receive a copy of this letter in the post, and anyone can request a copy from reception.

Dear Patients

We are changing when we invite patients for their annual review of certain medical conditions to your birthday month.

If you have a long-term condition such as Diabetes or Hypertension, you will receive an invitation to book your annual review with the practice during the month prior to your birthday month either by text or letter asking you to contact the practice.  When you do, our Reception Team will book you in for the appointment(s) you need to support your care.

A blood test appointment may be required for certain conditions, and this may also include a mini-health screen which will include your height, weight, pulse check, blood pressure, waist circumference and lifestyle information. Should any of these results show you need further care we will contact you to let you know.

For asthma and COPD, we will send you a questionnaire. A nurse will then review your responses and contact you, should any further action be necessary.

You may find that the period from your last review to your next review is more or less than 12 months as this enables us to transition to birthday month reviews over the course of 2023/24.  If you have any concerns about your condition before or after your review, please contact us to request a consultation.
