Referrals
If you have questions regarding a referral that has been made for you, you can:
- Telephone the number on your appointment letter (if you have one)
- Telephone RMS Kernow (Referral Management Service) on 01872 226700
- Telephone our medical secretaries on 01726 882745 on weekdays between 08:30-16:30.

RCHT Hotline
Treliske have now set up a telephone line and email address to support patients waiting for an appointment whose symptoms have worsened since their referral was made. When contacting them, your details will be taken by a call handler who will escalate your query to the appropriate speciality for advice and guidance.

Requesting a new referral
You can self-refer for the following services:
- Outlook South West offer psychological therapy services for people over the age of 16. You can self-refer for NHS funded therapies by contacting them on 01208 871905 or filling out their online self-referral form.
- Cornwall Healthy Weight can offer support with weight management, healthy eating and physical activity. You can self-refer by calling them on 01209 313419or filling out their online self-referral form.

Alternatively, if you need a referral to be made on your behalf for a service that you cannot self-refer to, please contact reception with your query.
